Drug-resistant tuberculosis (TB) has emerged as a grave health challenge in India. Recently, this issue was highlighted during the Maharashtra assembly session, where concerns about rising TB cases and the growing problem of drug-resistant TB were discussed. This issue is deeply troubling, and it's time we address it with urgency.

The primary concern with drug-resistant TB is that the bacteria no longer respond to standard treatments. This resistance forces patients to use more potent and toxic drugs, which can lead to severe side effects. While developed countries have made strides in managing TB through improved social conditions, India continues to struggle with more complex and harder-to-treat forms of the disease.
In India, treatment options for drug-resistant TB are limited and come with serious side effects. Patients may experience severe nerve damage, skin discoloration lasting up to three years, and even psychosis. These side effects make treatment a challenging and often discouraging process for patients, their families, and healthcare providers.
Hope in Form of Medication
Fortunately, recent advancements in TB treatment offer hope. The introduction of new drugs like bedaquiline in 2012 marked a significant breakthrough. These drugs have improved treatment outcomes for many patients. The more recent addition of pretomanid has further enhanced treatment success rates. In December 2022, the World Health Organization (WHO) recommended the BPaL regimen—comprising bedaquiline, pretomanid, and linezolid—as a new standard for drug-resistant TB. This regimen, often combined with moxifloxacin, has shown an impressive 89% success rate, compared to the 52% success rate of older treatments.
The BPaL regimen simplifies treatment significantly. Patients can complete their therapy in just six months, compared to the 18 to 24 months required by older treatments. This new regimen is safer, simpler, and more cost-effective, making it a viable option for widespread use. This underscores the need to expand access to the BPaL regimen in India. It should be incorporated into national treatment guidelines and widely available.
The Parliamentary Standing Committee on Health has also highlighted the need for faster adoption of new, safer treatments for drug-resistant TB. Additionally, there is a pressing need for improved diagnostic tools. Rapid point-of-care tests, similar to those developed for Covid-19, could significantly enhance early detection and treatment outcomes for TB. Addressing social-determinants like undernutrition that contribute to TB incidence is also crucial.
Our battle against TB is far from over. Success will depend on collaborative efforts from the government, scientists, physicians, and the community. We need to embrace new treatments and diagnostic innovations for TB tests. Together, we can make substantial progress in overcoming drug-resistant TB and move closer to a TB-free future.
